The location is amazing, it's literally in front of Fukushima station, but with a small plaza in between so it's not noisy at all. The breakfast was also really good. A nice mix of japanese breakfast (rice, miso soup, chicken, salmon, tea...) with a selection of occidental food (bread, croisants, coffee...). Of course, refillable buffet-style.
The room was nice, nothing extraordinary but it's japanese style so it's appreciated.
We got a smoking room without any of us smoking, and it smelled a bit. Not too much, only noticeable when you enter the room, but I personally dislike that smell.
Also in some corners of the room you could see some spider webs and some dust, sign that they don't really clean those spots frequently.

Staff was wonderful.
The location is perfect. I got the watch the trains from my room. Photo included of our view.
The hotel was quiet, or the soundproofing was good. Not sure which but we didn't have any issues with noise.
Breakfast was decent. We didn't take advantage of the other food offerings but it was nice that they existed.
I personally don't care if a hotel is dated as long as it's clean, but it should have been cleaner. There was mold in the bathroom and on the window sill. There was staining on the walls, bedframe, and bedding. Our room also had a faint BO smell. That said, I've paid more for worse stays and would consider returning.
